REVISED PHS 398 GRANT APPLICATION FORM NIH GUIDE, Volume 24, Number 19, May 26, 1995 P.T. 34 Keywords: Grants Administration/Policy+ National Institutes of Health The revised (5/95) Public Health Service grant application kit, PHS 398, will be available in late June. This revision, supersedes all previous editions. Applicants are to use the new application kits starting with the following dates: the September 1, 1995, receipt date for AIDS applications; the September 10, 1995, receipt date for NRSA Institutional Training Grant applications; and the October 1 and November 1, 1995, receipt dates for research grant and Research Career Award applications. Responses to requests for applications (RFAs) are to use the new form if the receipt date is September 1, 1995, or later. The revised PHS 398 application kit contains the forms for application and an instructional booklet that includes sample forms, definitions, assurances, and other relevant information. The instructional booklet should be retained for future use. Additional copies of the form pages can be obtained separately. The more important revisions are the following: applicant organization signature certifies compliance with all applicable assurances and certifications in the application; other support has been redefined and a format and sample is provided; several personnel items have been deleted for new applications; instructions have been expanded to accommodate all research career (K) awards.
